Eligibility and Enrolment
Below is an overview of the Plan’s key eligibility provisions and enrolment process. For complete details, please refer to the Eligibility and Enrolment section of the Pension Plan booklet or contact the Administrator.
Establishing Membership in the Plan
You are eligible to join the Plan as soon as:
- You are a member in good standing of Teamsters Local Union No. 155, and
- You are employed pursuant to the Master Agreement, and
- Your employer makes the required contributions to this Plan for your work, and
- You have completed and returned an enrolment form to the Administrator
How to Enrol in the Plan
Use the Teamsters Local Union No. 155 Pension Plan Application for Enrolment form to enrol in the Pension Plan and to select your beneficiaries.
Questions on completing the form can be directed to the Administrator.
Completed forms should be forwarded to the Administrator.
Once you are a Member of the Plan, all changes to Member information or beneficiaries should also be made by completing this form in full.
